EPI Info Survey Template

Epi Info is a powerful, free tool from the Centers for Disease Control (CDC) for public health professionals. You can use Epi Info to create a survey questionnaire, enter data into the questionnaire, analyze the survey data, and even create visual aids for your reports-all in one program! Epi Info also allows you to merge several databases into one. For example all counties in one region could conduct the Community Health Opinion Survey and combine the results to get regional as well as county specific statistics using Epi Info!

To save you time and assist you in managing your survey data, the Office of Healthy Carolinians/Health Education has created a template in Epi Info with all of the questions from the standard 2008 Community Health Opinion Survey, already entered for you. Download the "Epi-Info HC 2008 Survey File" by clicking on the link to the right.

You may modify the survey in any way to better fit your community. However, please remember not to enter any data into the Epi Info survey file until you are finished modifying your survey.

Before you begin using the Epi Info survey file:

  • Download the latest FREE version of Epi Info from www.cdc.gov/epiinfo/.
  • See the section below on Epi Info Training if you have never used Epi Info.
  • Download and read the "Community Health Opinion Survey" and decide if there are any questions you would like to modify.
  • Download and read the "Instructions for the Epi Info Survey Template File" for more information about how to modify and enter data into the survey template.
  • Download and look over the "Epi Info Codebook," which will help you understand the variable names during data analysis.

Epi Info Training

If you are new to Epi Info, please take advantage of one of the following training opportunities to familiarize yourself with the program before using it:

  • A free, self-guided tutorial on using Epi Info for Community Health Assessment is available at: www.cdc.gov/epiinfo/tutorials.htm
  • Free online Epi-Info trainings on topics for all levels of users at http://nccphp.sph.unc.edu/training/trainings.php
    • Click on "Trainings by Topic," then "Software for Public Health," then "Epi Info"
    • Scroll to the bottom to see the trainings Epi Info 1-6 for beginners
  • Epi Info regional onsite trainings can be arranged by request. Please contact your Healthy Carolinians regional consultant for more information.
