Columbus County
Healthy Carolinians is a community-based partnership that coordinates
and facilitates the "process" of "connecting"
county-wide health initiatives in an effort to "build the capacity"
by pooling resources, eliminating duplication and ultimately creating
an environment that supports "total health" and an improved
"health status" for Columbus County.

Chronic
Disease
Heart Disease and Stroke: Reduce the burden of cardiovascular
disease by providing in-service training for health care providers,
community education, community screenings and policy initiatives.
Overweight/Obesity: Raise awareness of the prevalence of
obesity in the community, by promoting healthy eating and physical
activity in children and their families through culturally relevant
social marketing interventions that are designed to effect behavioral
change.
Diabetes: Increase awareness of diabetes complications by
promoting the ADA certified Diabetes Education program at Columbus
Regional Healthcare System, Inc.
Health Promotion
Physical Activity and Nutrition: Increase healthy habits
by establishing community-based fitness sites; integrate fitness
and nutrition activities in after-school programs; providing community
education on benefits of exercise and healthy eating; and establishing
physical activity opportunities at churches, schools and community
sites.

Economic Opportunity: Facilitating "Discover Columbus"
to help in eliminating income inequalities among different segments
of the population and ensure that ALL communities have a viable
and sustainable economy and individual members have the opportunity
to participate fully in work and production.

1. Expansion
of the Diabetes Self-Management Classes: Development into an ADA
Diabetes Education program at Columbus County Hospital. The American
Diabetes Association recognizes this education service as meeting
the National Standards for Diabetes Self-Management Education.
2. Public Awareness Campaigns, Presentations to Businesses and
Civic groups
3. Two-day County-wide Health Expo: Involved MIRT (Medical Innovative
Readiness Training) team from the National Guard (2000 health
screenings throughout the county)
4. Developing Community based programs utilizing trained "lay
health educators-community encouragers" to promote Cardiovascular
Health, Early
Recognition/Treatment of Acute Coronary Syndromes, and Development
of an Enhanced Dissemination Utilizations Center (EDUC) for the
region
5. Walking trails (signage working with the Health Dept and Mark
Gilchrist) & Forestry Museum Tree Trail - working with the
NC Museum of Forestry in Whiteville, NC
6. Facilitating "Discover Columbus: Program for the Rural
Carolinas" by investing in asset-building, employment-generating
and income-building rural projects and EITC Campaign-(Earned Income
Tax Credits), funded by The Duke Endowment. (working with www.mdcinc.org)
