  • Kids in the Garden (3rd graders come to our community garden to plant seedlings they grew, go to stations (vermiculture, composting/rain barrels, food safety/tasting, planting in the garden, planting an herb to take home with herb storytime) Kids are also fed lunch-vegetable pizza and will receive a Kids in The Garden T-shirt.
  • Fruits & Veggies Nutrition Presentations--children at various child cares and community groups aged 3-6 watch a short 10 minute video on fruits and veggies, receive stickers, play with fruit and veggie seedies and then are read 3 books on fruits and veggies. 14 nutrition related books are left in their classroom as well as two posters on nutrition. All children receive a packet to take home with a coloring book on nutrition, a gel bracelet, etc. Additionally, informational packets are sent home to parents on portion size and nutrition.
  • Beaufort Healthy Carolinians has recently completed the PSJ/JSS ESMM Walking Trail (0.98 miles of crushed rock packed with a mechanical packer on the campus of two of our public schools).
What health disparities is your partnership working on?
We are working on ways that low-income people can participate in physical activity in our community. We hope to soon work on more walking trails and have another event that will be able to the community to learn how physical activity can benefit them. This will also help the uninsured population. Our group is currently partnering with other local agencies to do community gardening which can affect many areas of health.

Bladen HealthWatch is increasing cardiovascular, diabetes, stroke, and cancer screenings at worksites, churches, schools, and in communities.

The initiative is increasing by 10% the rate of self-supported physical activity, and fruits/vegetable consumption. Bladen healthWatch is decreasing by 10% the rate of self-reported tobacco use and unhealthy weight.

The initiative has created a School Health committee to address access of child health services, identify needs, and develop integrated services and referrals. Bladen HealthWatch has sought funding to add nurses in the public school system, trained educators to recognize and address children with chronic health issues, and developed a faith-based program to identify children in need.

Bladen HealthWatch has created a marketing strategy that publicizes the initiative, provides resources through the public library, promotes interagency communication, and funds a Parish Nurse to develop a health ministry.

Bladen HealthWatch has initiated a Safe Kids Coalition, developed brochures, implemented countywide campaigns to educate the public about safety issues for children, and engaged in local advocacy for the enactment and enforcement of policies and laws that address child safety.
What health disparities is your partnership working on?
Over 32,000 adults aged 40+ and school aged children with chronic diseases or exhibiting risk factors for chronic diseases are underinsured and uninsured. The program seeks to reach 88%, (28,000).

The Alliance was recognized by the Cleveland County Business Development Corporation during Minority Enterprise Week as the Community Advocate Award Winner in 2007. Women's preventative health clinic launched a male health initiative titled "Man Up". Man Up is a social marketing campaign targeting males 15 — 24 years of age to increase their accessibility to reproductive health services at the health department.

The Health Department partnered with the Cumberland County HIV task force to promote a billboard campaign promoting HIV testing. Ten billboards were rotated throughout the county.

The LiveWell Granville Health Promotion Workgroup created a Greenway Master Plan (2006) for the County and constructed the first ½ mile of greenway in the county (2007). Work is underway to develop ordinances and other strategies to further implement the plan and thereby encourage residents to Move More.

The Youth Issues Workgroup purchased books (English & Spanish languages) for a summer reading bookmobile and 67 hours of tutoring through a summer reading program offered through the Oxford Housing Authority (2008). To promote academic excellence, the Youth Issues Workgroup started a ‘B' Average Initiative which challenges students (3rd through 12th grade) served by the Oxford Housing Authority to earn good grades, and rewards them with prizes (gift cards, pizza party, etc…) for their academic effort.

As part of an Eat Smart Move More Community Grant, the Health Promotion Workgroup joined forces with Vance County partners to fund mini grants to promote policy and environmental change that supports healthy lifestyles. The Granville County grantees were Oxford Park & Recreation serving the county, Grassy Creek Baptist Church serving African-Americans, Christian Faith Center serving African-Americans, R.J. Blackley Drug & Alcoholic Treatment Center, Central Regional (Psychiatric) Hospital serving employees and patients.
What health disparities is your partnership working on?
The Youth Issues workgroup is focusing on dropout prevention which currently impacts African-American male and female students, Hispanic students and generally limited-resources audiences. The B Average Initiative at the Oxford Housing Authority is primarily reaching low-income African Americans. The Health Promotion Workgroup (which has merged with Vance County's to form "Vance-Granville WoW! - Working on Wellness") chose not limit its focus because chronic disease affects all citizens. However, through the mini-grant program 2 grantees are African American churches and 2 are work sites, of which one is staffed primarily by African American females and while the staff of the other represents no particular demographic, the project also serves the patients who are mentally ill.

M.I.R.A.C.L.E. (Managing Insulin Resistance, A Changing Life Experience)
Jones County Healthy Carolinians has developed M.I.R.A.C.L.E., a free diabetes education program. This church-based health literacy program provides prevention and management education workshops, food literacy, blood pressure and weight screening education and one-on-one screening and education sessions for persons with diabetes or at-risk for diabetes. The goal of this project is to increase the capacity within Jones County to prevent, diagnose and medically manage diabetes among African Americans. Because Jones County has such a large faith based minority population, key to the implementation of this project is a population-based strategy focusing on church involvement in each of the communities where many individuals age 18 and older are at risk for type II diabetes.

H.E.A.R.T.S. (Healthy Enthusiastic Active Responsible Teachers and Staff)
H.E.A.R.T.S. is a general wellness program for the teachers and staff of Jones County that focuses on nutrition and physical activity.

Annual Walk 4 Your Health Walk-A-Thon
Jones County Healthy Carolinians developed the annual Walk 4 Your Health walk-a-thon. Walk 4 Your Health is working to increase physical activity among the adult population in Jones County.

Jones County Employee Wellness Program
Jones County Healthy Carolinians developed the Jones County Employee Wellness Program.  This program promotes healthy weight loss for each participant by increasing their daily physical activity and encourages healthy snacking while at work. 

Annual Health Summit
The Annual Health Summit was developed by Jones County Healthy Carolinians to provide an opportunity for county residents to voice their community concerns to the staff of the Jones County Health Department and other community organizations.
What health disparities is your partnership working on?
Our partnership is currently working to address health disparities seen in the African American community related to diabetes. Health Disparities related to poor nutrition and lack of physical activity experienced among lower income populations are also being addressed.

Health Promotion adopted a healthy eating policy at the health department. The health department now has refrigerator and trays stocked with healthy snacks and beverages for the employees to buy whiled they are at work.
What health disparities is your county working on?
African Americans have higher death rates from heart disease, cancer, HIV, diabetes, homicide, and stroke compared to whites. Lenoir County's statistics document higher than state averages in most of these areas. I am working on the heart disease with my two policies.

  • HCOC established the Advocates for Adolescents: Coalition to Prevent Underage Drinking and Drug Use in northern Orange County in collaboration with Community Back Yard, a program of the Mental Health Association in Orange County. The Coalition aims to implement environmental and policy change strategies to limit youth's access to and the availability of alcohol and prescription drugs. Some activities of the Coalition include a Safe Homes Network, development of a Youth Coalition, and alcohol purchase surveys.   • Through the Pediatric Obesity Initiative for Clinicians and the POI for School Nurses, HCOC provided child health care professionals and school nurses clinical tools, practical support, capacity building training, and guidance to help improve care and outcomes for overweight youth.
  • Lead by elementary Physical Education instructors and More at Four/Head Start/Pre-K teachers, teams of students completed a Healthy Classroom Challenge. Teams create original routines that incorporate the ESMM messages and physical movement. Groups performed at the annual Healthy Kids Day event and reinforced these messages with families and the Orange County community.
  • Twelve local partners (businesses, churches, and local organizations) participated in the Orange County Eat Smart Move More (ESMM) Consistent Messaging Campaign. Partners promoted ESMM messages through new and existing communication channels to encourage individuals, families, and children to make positive behavior changes around physical activity and healthy eating. Articles and ads were published in local newspapers to promote the "message of the month."
  • HCOC developed an Orange County Recreation Map that includes all of the public recreation areas and trails in the county encompassing over 60 locations administered by 15 different organizations.
  • HCOC hosts an annual community event, Real Men Rock, to raise awareness about Shaken Baby Syndrome and child abuse prevention. Child ID kits are provided by the Sherrif's office.
  • In partnership with the Period of PURPLE Crying®, HCOC held capacity building training workshops on Shaken Baby Syndrome prevention for home and child care providers; health department nurses and home visiting staff.
  • HCOC hosted over 22 Brown-Bag-Lunch and 14 half-day training sessions for the community and professionals on how to recognize and respond to child abuse prevention, domestic violence, and mental health.

  • Increase access to mental health services for the under and uninsured;
  • Reduce underage drinking and drug use in northern Orange County (a part of the county with fewer resources for youth); and

  • Development of ENC Regional Stroke Network to improve quality and outcomes of stroke.
  • '"What's In Your Red Bag?'" Medication Safety program developed- 2000 bags have been purchased and are given out during educational sessions to help seniors keep their medications organized.
  • The Cornerstone Ministry, Inc has been awarded grants from the NC Health and Wellness Trust Fund to reduce the racial disparity in cardiovascular disease morbidity and mortality in Pitt County.
  • The Pediatric Healthy Weight Case Management Program was developed to help Pitt County children and parents find the care and resources to reach and stay at a healthy weight. The program works to: raise awareness of childhood obesity, ensure children throughout the county receive a consistent level of care and support policies that promote physical activity and good eating habits. The program is supported by grant funds from the Duke Endowment.

  • Prostate Cancer Screens- in observance of National Prostate Cancer Awareness month each September local urologists and other medical provider volunteers conduct a free prostate screen for men ages 40-70. This year over 300 men were screened for prostate cancer.
  • Project H.E.A.L.T.H. - (Healthy Eating and Active Lifestyles for Tomorrow's Health) focuses on nutrition education efforts by designing and implementing programs that increase fruit and vegetable access, awareness and consumption. Due to the innovative approaches, Project H.E.A.L.T.H. program was honored with the 2008 N.C. Fruits and Veggies Nutrition Coalition's Gold Star Award for Health Care.
  • Diabetes Education- the Diabetes Community Center of Robeson County offers comprehensive diabetes education program for all types of diabetes (type 1, type 2, gestational and pre-diabetes). The program is recognized by the American Diabetes Association.
  • Injury Prevention- partnering with local Safe Kids Coalition (Safe Kids Robeson County), Robeson County Church & Community Center and the Robeson County Partnership for Children to provide bike helmets for needy children who are the recipients of free bicycles donated by Campbell's soup during the month of December.
  • Maxton TOP (Teen Outreach Program) -- allow youth in the town of Maxton an opportunity to enhance their personal development through an exploration of their values and relationships with partners, peers and community.

  • Vance-Granville WoW (Working on Wellness) just completed a successful mini-grant program funded by ESMM...NC. 6 Vance County organizations implemented sustainable policy or environmental changes to encourage residents to eat smart and move more, while VC Commissioners instituted an annual Eat Smart Move More County Vance County Award program in July.
  • VCHD applied for TPPI funding to address Teen Pregnancy in the schools but was denied. APPCNC has since awarded funds to build local capacity to address this issue. VCHD is currently working with area partners on the feasibility of starting a local Girls on the Run Affiliate with the goal of sponsoring at least 1 team in the coming school year and building the program in years to come. VCHD will be working with APPCNC and VC Schools to implement and train the appropriate staff on the "Making Proud Choices" in the schools to accommodate the new legislation which allows for comprehensive sex education.
  • Through on-going community programs such as Community Service/Restitution, Project Youth Outreach, Friends of Youth and Conflict Management Services; the local JCPC is actively engaged to help reduce substance abuse rates, gang issues, anger / conflict management issues, and high school drop out rates. Upon request the Vance County Police Department facilitates drug and gang violence presentations. The JCPC also voted to support the Judicial Advisory Council which operates to address the needs of families in violation of the Compulsory Attendance Law without resorting to judicial action.
What health disparities is your county working on?
The workgroup addressing chronic disease is focusing on prevention efforts which they believe ought to be community—wide. The mini-grant project reached school-aged youth, the mentally and physically handicapped, seniors, an African American church, and YMCA non-members.

Teen pregnancy efforts address school-age youth. While many counties have wide-disparities between the races, Vance County's white teen pregnancy rate is increasing while the non-white rate has decreased. Hence we intend to target teens of all races with our efforts.

Drug use and gang issue efforts are directed at youths (and their families) at-risk for adjudication and services through the local Juvenile Crime Prevention Counsel and local law enforcement.

  • Distribute car seats to people who attend childbirth prep class. Car seats are provided free for Medicaid participants and are provided at a reduced cost for others.
